Specimen Requirements

Isolate infecting bacteria. Bacteria must be in pure culture, actively growing, placed in a proper mailing container, and labeled as an etiologic agent. (Agar plate is not acceptable.)

Note:  1. Label container with patient’s name, date of birth or hospital medical record number, specimen source, date and time of collection, and collector’s initials.

2. Specimen source is required.
