Useful For

Serum creatinine measurement is used in estimating GFR for people with chronic kidney disease (CKD) and those with risk factors for CKD (diabetes, hypertension, cardiovascular disease, and family history of kidney disease)

  • Diagnosing and monitoring treatment of acute and chronic renal diseases
  • Adjusting dosage of renally excreted medications
  • Monitoring renal transplant recipients

Specimen Requirements

Special Note:  Plasma or serum must be removed from cells within 4 hours of collection

Preferred:  Green-top (lithium heparin) gel tube 

Acceptable:  Plain, red-top tube or SST

Specimen Volume: 2 mL

Pediatric (minimum) volume: 0.5 mL

GFR CALCULATED (mL/min/1.73 m2)

  • Stage 1:  ≥ 90 mL/min
  • Stage 2:  60 - 89 mL/min
  • Stage 3:  30 - 59 mL/min
  • Stage 4:  15 - 29 mL/min
  • Stage 5:  ≤14 mL/min

When creatinine is ordered, a GFR will automatically be calculated on all patients >18 years old.

The MDRD GFR formula is only valid for adults between ages 18 and 85.

Interferences

  • Use of this testing is not recommended for patients undergoing treatment with phenindione, due to the potential for falsely depressed results.
  • N-acetyl-p-benzoquinone, a metabolite of acetaminophen, in concentrations of >10 mg/L, correlating to toxic levels of acetaminophen, may lead to falsely depressed results.

  • Glomerular Filtration Rate (GFR), Estimated: Calculated on patients between 18 and 85 years old.
  • CKD-EPI 2021 equation:  GFR = 142 x min (Scr/K, 1) x max (Scr/K, 1)-1.200 x 0.9938age x 1.012 (if non-male)
