Important Note

Order Fungal Smear (FS) if the source is a mouth swab.

To determine presence of fungal elements in suspected specimens.

Submit only 1 of the following specimens:

Hair, Nail Cuttings, or Skin Scrapings- collect with sterile instrument and place on Dermapak card

Instructions for Dermapak card

a. Ensure the card is not expired

b. Lift Flap A and place specimen directly on central black area

c. Close Dermapak by folding flap A

d. Fold in flaps B and C and remove white backing strip from adhesive

e. Fold flap D and press down firmly

f. Properly label specimen

g. Place labeled Dermapak in provided bag and place in biohazard bag for transport to hospital site within 24 hours
